Confidential Information Form (INFO)

Figure 1 shows the template assembled into an interview after ending the interview for Petition for Dissolution of Marriage.

When the template opens in Figure 1 the first dialog selected in the left Interview Window, is the "County of Lincoln" dialog , which we had already answered in the Petition interview.  Notice all the textboxes are answered in the right dialog Window.

Next, we will view the second dialog , Confidential Information Form (INFO) shown above in Figure 2 the Interview Window.

By default, if you have completed the Petition dialog as we have in this demonstration the first option list will have "Divorce / Separation / Invalidity / Nonparental Custody / Paternity / Modifications" selected.

Also, while answering the questions in the Petition dialog , if you responded in Paragraph 1.12 Protection Order to "The court should grant the domestic violence and antiharassment petition for order for protection:" question the appropriate options would be selected by default. Figure 3 shows a Petition dialog has been answered and in this sample, "domestic violence" was selected in Paragraph 1.12 Protection Order.

Many dialogs have at least one question in red preventing the user from moving to the next dialog without making a selection, this assures the user will not overlook a dialog without making a conscientious decision to do so. In the dialog shown above in Figure 2, we chose to require the user to respond to the following: "A restraining order or protection order is in effect protecting the:"; yet,most of the time the response shall be "None of the Above". In addition, the following three questions are defaulted to "No".   Changing these answers to "Yes" will require more input from the user as shown in Figure 3.

The remainder of the dialogs in the Interview Window have previously been answered in the Petition for Dissolution of Marriage demonstration we have been creating.  However, they are available if you require the ability to update information or make changes.

Figure 4 and Figure 5 show the finished forms in the Document Preview tab. An important note, if there are more than two children, such as three or four, an additional Addendum Confidential Information Form would be automatically added, and as would another if, there are five or six children.
